  • At-Home Antigen Test Opt-In Form for MPS Students

    The Milford Public School District in partnership with the Department of Secondary and Elementary Education will provide each student with home antigen tests free of charge.  This program is completely voluntary.  If you want your child to participate, you will need to fill out a separate form for each child that attends the Milford Public School District.  Students will pick them up bi-weekly from their school.  Each kit will contain 2 tests.  The intent is to have students that opt in to test once a week at home.

  • American Rescue Act Funding (ARP): ESSER III

    The American Rescue Plan Act is the latest COVID-19 relief package to provide funding for school districts. Milford Public School Schools (MPS) will receive funds based on the same proportion under the Elementary and Secondary Education Act (ESEA) Title-IA. Schools are required to reserve at least 20% of the funding they receive to address learning loss. The remaining funds are flexible and can address various needs related to COVID expenses through September 30, 2024 (read more)

Return the completed form to the address listed on the form. If you already have a family portal account you do not need to fill out this form.
